FibroScan & Elastography: Liver Stiffness vs Liver Fat

FibroScan measures liver stiffness (a fibrosis surrogate, in kPa) and CAP measures fat. How to read elastography, why fat and fibrosis aren't the same, MRE, and the limits of the test.

A FibroScan measures liver stiffness in kilopascals (kPa) — a surrogate for fibrosis (scarring) — using a painless probe. The same device reports CAP, a separate estimate of liver fat. So one visit gives two different numbers: stiffness for scarring, CAP for fat. Higher stiffness suggests more fibrosis, but the test is an estimate, not a biopsy.

The one distinction to hold onto: fat is not fibrosis

Elastography is where the fat-versus-fibrosis distinction becomes concrete, because a single FibroScan report hands you two numbers that mean completely different things:

  • Liver stiffness (kPa) — a physical surrogate for fibrosis (scarring). This is the number that speaks to risk.
  • CAP (Controlled Attenuation Parameter, dB/m) — an estimate of liver fat (steatosis).

Patients (and occasionally reports) blur these. A high CAP means a fatty liver; it says little about scarring. A high kPa suggests fibrosis; it isn't a measure of fat. Liver fat and liver fibrosis are measured differently because they are different things — the central rule of this whole section. → What Is MASLD?


What VCTE / FibroScan actually does

FibroScan is the common brand name for VCTE — vibration-controlled transient elastography. A probe placed on the skin over the liver sends a gentle mechanical pulse and measures how fast the resulting shear wave travels through liver tissue. Stiffer tissue transmits the wave faster. The device converts that speed into a stiffness value in kPa.

It is quick (minutes), painless, radiation-free, and done at the point of care. It measures a physical property of the liver rather than estimating from bloodwork — which is why it's more informative about fibrosis than FIB-4, and why it sits after FIB-4 in the pathway. → The W8Experts Diagnostic Approach


How liver stiffness (kPa) is interpreted

Higher stiffness means a higher probability of significant fibrosis. Interpretation is probabilistic and context-dependent, not a fixed line, and thresholds vary by cause of liver disease and by lab:

  • Lower kPa (roughly <8 kPa in MASLD) — advanced fibrosis is unlikely; strong reassurance, especially with a reliable reading.
  • Intermediate kPa (roughly ~8–12 kPa) — a grey zone; may prompt repeat testing, MRE, or specialist input.
  • Higher kPa (roughly >12 kPa, and >15 kPa often used to flag compensated advanced chronic liver disease) — raises concern for advanced fibrosis; usually prompts hepatology referral and further evaluation.

Treat those numbers as a rule-of-thumb for MASLD, not fixed lines. Two honest caveats: first, kPa is a surrogate — it correlates with fibrosis but isn't a direct scar measurement. Second, the exact cutoffs differ between guidelines, centres, and cause of liver disease, so a specific number should be read against the reporting lab's ranges and your whole clinical picture, not a generic table. And remember the other number on the report: CAP measures hepatic fat (steatosis, in dB/m), not fibrosis — it says nothing about scarring. Any specific threshold should be checked against current guideline/manufacturer values for the relevant disease.


How CAP (fat) is interpreted

CAP estimates how much the ultrasound signal is attenuated by fat — a proxy for steatosis. A higher CAP means more liver fat. It's useful for confirming and roughly grading fat, and for tracking change, but it is not a fibrosis measurement and shouldn't be read as one. For precise fat quantification, MRI-PDFF is more accurate; CAP is the convenient bedside version. → Diagnostic Approach


MRE — the more accurate cousin

MRE (magnetic resonance elastography) measures liver stiffness using MRI rather than a handheld probe. It maps stiffness across much more of the liver and is more accurate than VCTE and less affected by body habitus — but it's costlier, less available, and requires an MRI scanner. Its usual role is for discordant or uncertain cases (for example, an unreliable FibroScan in a person with obesity, or results that don't fit the clinical picture), and in research. Like VCTE, it's a stiffness surrogate, not a biopsy.


The limits — when elastography is unreliable

Elastography is powerful but not infallible. Stiffness can be over- or under-estimated by factors that have nothing to do with permanent scarring:

  • Obesity and probe fit. A thick chest wall can make standard-probe readings unreliable or unobtainable; an XL probe helps, and MRE may be needed when FibroScan fails. This matters because the people most likely to have MASLD are also the most likely to challenge the probe.
  • Recent food. Eating increases liver blood flow and can transiently raise stiffness, so testing is usually done fasting (commonly ~3 hours).
  • Hepatic congestion / heart failure. A congested liver is stiffer independent of fibrosis, inflating kPa.
  • Acute hepatitis / marked inflammation, cholestasis, and heavy alcohol use can all raise stiffness.
  • Operator technique and an inadequate number of valid readings reduce reliability; reports include quality metrics (e.g., number of valid measurements and variability) that should be checked.

Where elastography sits: FIB-4 first, then stiffness

Elastography is second-line, deployed when the cheaper first-line filter (FIB-4) is indeterminate or high:

SIGNATURE — "What the evidence says: Elastography"

  • What we know: VCTE/FibroScan liver stiffness is a validated fibrosis surrogate that meaningfully improves risk stratification after FIB-4; CAP estimates fat separately; MRE is the most accurate noninvasive stiffness measure.
  • What we think: Used as a second-line test with attention to reliability, elastography safely reduces the need for biopsy.
  • What we don't know: The single best universal cutoffs, and how reliably repeated stiffness tracks true fibrosis change over time — especially in obesity.
  • What patients should do: Read the two numbers separately (kPa = scarring surrogate, CAP = fat), fast beforehand, and interpret any result with a clinician and its reliability metrics — not as a standalone verdict. → FIB-4 Explained

Questions patients ask

What does a FibroScan measure?

Liver stiffness in kPa — a surrogate for fibrosis (scarring) — plus a separate CAP number that estimates liver fat. Two numbers, two different questions. Grade A 🟢

Does a FibroScan measure liver fat or scarring?

Both, separately. The kPa stiffness value is a fibrosis surrogate; the CAP value estimates fat. Don't read a high CAP as scarring or a high kPa as fat. Grade A 🟢

What does the kPa number mean?

Higher stiffness suggests more fibrosis, but it's probabilistic and the cutoffs depend on the cause of liver disease and the lab. It's interpreted with your whole picture, not against a universal line. Grade B 🟡

Is a FibroScan the same as a FIB-4?

No. FIB-4 is a blood-based estimate that comes first; FibroScan physically measures liver stiffness and comes second, when FIB-4 is indeterminate or high. Grade A 🟢FIB-4 Explained

Is a FibroScan the same as an ultrasound?

It uses ultrasound technology but measures stiffness (and CAP for fat), which a standard diagnostic ultrasound does not. A regular ultrasound detects fat, not fibrosis. Grade A 🟢

Do I need to fast before a FibroScan?

Usually yes — recent food raises liver blood flow and can transiently increase stiffness, so testing is typically done fasting (often about 3 hours). Grade B 🟡

Can obesity make a FibroScan inaccurate?

My CAP is high — should I be worried about scarring?

A high CAP means a fatty liver, not scarring. Fibrosis risk is judged from the kPa stiffness value and FIB-4, so a high CAP alone isn't a fibrosis result. Grade A 🟢

Does a normal FibroScan mean my liver is fine?

A reliable, low stiffness reading is reassuring for advanced fibrosis, but it doesn't erase the underlying metabolic disease driving the fat — which still needs attention. Grade B 🟡What Is MASLD?

How often should a FibroScan be repeated?

It depends on your risk and whether you're being treated; intervals are individualized. Trends over time can be informative, but repeat testing is guided by a clinician. Grade C 🟠
