Semaglutide (Ozempic, Wegovy): How It Works, Results & Risks

Semaglutide explained by endocrinologists: how much weight loss to realistically expect (with trial context), cardiovascular and liver evidence, side effects, and what happens if you stop.

In the STEP 1 trial (adults with obesity, no diabetes, 68 weeks), semaglutide 2.4 mg produced about 15% mean total body-weight loss, versus ~2.4% on placebo. Roughly a third of participants lost ≥20%. But this is an average — individual results vary widely, depend on tolerating the full dose, and weight is largely regained if the drug is stopped.

What semaglutide is

Semaglutide is a GLP-1 receptor agonist — a once-weekly injectable (also now an oral tablet) that mimics a natural gut hormone. The same molecule is sold under different names for different uses: Wegovy for weight management, Ozempic for type 2 diabetes. They are not different drugs; the confusion is mostly branding and dosing. → GLP-1 hub for the class mechanism.

Formulations: - Injectable, once weekly — Wegovy (up to 2.4 mg) and Ozempic (up to 2.0 mg). - Oral, once daily — Rybelsus (3–14 mg, diabetes) and, newly, an oral 25 mg tablet approved for weight management — the first oral GLP-1 for obesity, giving a needle-free option with efficacy in the range of the injection in its pivotal trial.


How it works

Semaglutide acts on GLP-1 receptors in the brain and gut to increase fullness, reduce hunger and food preoccupation, slow gastric emptying, and improve glucose control (stimulating insulin only when glucose is elevated, so it rarely causes low blood sugar by itself). It targets the appetite biology that defends weight — the reason it works where willpower often can't. → Why Weight Is So Hard to Lose


What the trials actually show

Weight (obesity, no diabetes) — STEP 1, 2.4 mg, 68 weeks: ~15% mean total weight loss vs ~2.4% placebo; about a third lost ≥20%. Placebo-adjusted ≈ 12–13%.

Weight (type 2 diabetes) — STEP 2, 2.4 mg, 68 weeks: ~9–10% mean — notably less than in people without diabetes, a consistent pattern that patients with diabetes should expect.

Maintenance — STEP 4: people who continued semaglutide kept losing/maintained; those switched to placebo regained weight, consistent with the chronic-treatment framing.

Cardiovascular — SELECT: in adults with established cardiovascular disease and overweight/obesity without diabetes, semaglutide 2.4 mg reduced major adverse cardiovascular events (heart attack, stroke, CV death) by about 20% over ~3 years. Liver — ESSENCE: in noncirrhotic MASH with fibrosis, semaglutide 2.4 mg improved MASH resolution and, to a lesser degree, fibrosis, supporting its FDA approval for F2–F3 MASH. Important nuance: reducing liver fat and resolving inflammation is not the same as reversing scarring. → GLP-1 & Fatty Liver

Oral 25 mg (OASIS program): produced clinically meaningful weight loss supporting approval for weight management.

Side effects

Common (mostly GI, dose-related, usually improve): nausea, vomiting, diarrhea, constipation, reflux, burping, fatigue. Managed with slow titration, smaller and lower-fat meals, and hydration.

Less common / serious: gallstones and gallbladder disease (more likely with rapid weight loss), pancreatitis (rare), dehydration/kidney injury from severe GI symptoms, injection-site reactions. "Ozempic face" is loss of facial fat from weight loss itself, not a specific drug toxicity. Hair thinning, when it occurs, is typically the telogen effluvium seen with any rapid weight loss and usually recovers. Contraindications & cautions

  • Personal or family history of medullary thyroid carcinoma, or MEN2 — contraindicated (boxed warning; based on rodent C-cell tumors, not established in humans). → Do GLP-1s cause thyroid cancer?
  • History of pancreatitis — caution.
  • Active severe GI disease / gastroparesis — caution.
  • Pregnancy — not recommended; discontinue when pregnancy is planned or confirmed (see below).
  • Type 1 diabetes / history of diabetic retinopathy — specific cautions in diabetes care.

Monitoring, pregnancy and surgery

  • Monitoring: response (weight, glucose/A1c), GI tolerance, and — because muscle preservation matters — attention to protein intake and resistance training. → Exercise & Muscle
  • Pregnancy/lactation: stop before conception (the label advises discontinuing ~2 months before a planned pregnancy given the long half-life). Not for use in pregnancy. Note GLP-1s can reduce oral-contraceptive reliability during dose changes for some formulations — discuss contraception. → Women's Health & Obesity
  • Perioperative: because semaglutide slows gastric emptying, anesthesia guidance has evolved around holding GLP-1s before surgery/procedures to reduce aspiration risk. Follow your surgical/anesthesia team's current instructions.

Stopping semaglutide and weight regain

In STEP 1 extension data, people who stopped semaglutide regained roughly two-thirds of lost weight within about a year, and cardiometabolic improvements largely reversed. This is the defended set point reasserting itself — not a personal failure. It's the central reason we treat semaglutide as chronic-disease therapy and plan maintenance (including possible lower-dose continuation) before starting. → Weight Regain & Maintenance


Unanswered questions

  • Best long-term maintenance dose after target weight.
  • Decade-plus safety and durability.
  • Full effect on liver fibrosis (vs fat and inflammation).
  • Long-term muscle and bone outcomes, especially in older adults.

Questions patients ask

Is Ozempic the same as Wegovy?

Same molecule (semaglutide), different brand and approved dose — Ozempic for diabetes, Wegovy for weight. Grade A 🟢

How fast will I lose weight?

Gradually over months as the dose is increased; trials ran 68 weeks. Fast early loss isn't the goal — sustainable loss with muscle preserved is. Grade A 🟢

Does the pill work as well as the injection?

The oral 25 mg weight-management tablet showed efficacy in the range of the injection in its trial; individual choice depends on preference, tolerability and access. Grade B 🟡

Will I have to take it forever?

Weight is largely regained if you stop, so most benefit requires ongoing treatment — sometimes at a lower dose. Some maintain with intensive lifestyle support. Grade A 🟢

Can it help my fatty liver?

Does it cause muscle loss?
